Advanced Oxygen Therapy (Topical & Hyperbaric)
Oxygen therapy represents a powerful adjunctive treatment enhancing tissue oxygenation to support healing in hypoxic wounds and conditions with compromised circulation. Our comprehensive oxygen therapy program includes both topical oxygen delivery systems and hyperbaric oxygen therapy (HBOT), providing a range of options tailored to specific clinical needs and patient circumstances.
We operate a state-of-the-art hyperbaric medicine department with multiplace and monoplace chambers, complemented by advanced topical oxygen delivery systems for outpatient and home use. Our program follows evidence-based protocols for approved indications while maintaining the highest safety standards through rigorous training, comprehensive monitoring, and meticulous protocol adherence.
Hyperbaric Oxygen Therapy (HBOT)
We provide HBOT at 2.0-2.4 atmospheres absolute (ATA) for FDA-approved indications including diabetic foot ulcers, refractory osteomyelitis, necrotizing soft tissue infections, compromised grafts/flaps, radiation tissue injury, and carbon monoxide poisoning. Our protocols include precise treatment pressures, durations (60-120 minutes), and frequencies (daily to 5x/week) based on specific indications and response.
Topical Oxygen Delivery Systems
We offer continuous topical oxygen therapy (TOT) and cyclical topical oxygen therapy systems for outpatient and home use. These portable devices deliver 100% oxygen directly to wound surfaces at controllable flow rates, creating a localized oxygen-rich environment without systemic effects. Ideal for patients unsuitable for HBOT or requiring extended treatment durations.
Comprehensive Wound Assessment & Monitoring
We perform transcutaneous oxygen measurements (TcPO2) at baseline and periodically during therapy to document oxygenation improvement. Our monitoring includes wound measurement tracking, photographic documentation, and regular assessment of treatment response using validated tools. We coordinate with referring providers to integrate oxygen therapy with comprehensive wound management.
Safety Protocols & Patient Education
Our hyperbaric safety program includes comprehensive patient screening (contraindication assessment), detailed education on chamber procedures, strict fire safety protocols, emergency procedures, and continuous monitoring during treatments. For topical systems, we provide thorough training on home use, maintenance, and recognition of potential issues requiring medical attention.

FDA-approved indications include diabetic foot ulcers with hypoxia (TcPO2 <40mmHg), refractory osteomyelitis, necrotizing soft tissue infections, compromised grafts/flaps, radiation tissue injury (osteoradionecrosis, radiation cystitis/proctitis), carbon monoxide poisoning, arterial gas embolism, and decompression sickness. Off-label uses require careful consideration and documentation.
HBOT increases dissolved oxygen in plasma 10-15 times normal, reaching hypoxic tissues despite poor circulation. This enhances fibroblast proliferation, collagen synthesis, angiogenesis, leukocyte bacterial killing, and antibiotic effectiveness while reducing edema through vasoconstriction. The intermittent hyperoxia also stimulates stem cell mobilization and growth factor release.
Treatment courses vary: diabetic foot ulcers typically require 30-40 treatments, radiation injuries 30-60 treatments, necrotizing infections 10-20 treatments. We assess response after 10-15 treatments using objective measures (wound size reduction, TcPO2 improvement) and adjust treatment plans accordingly. Most protocols involve daily treatments 5-6 days per week.
HBOT risks include barotrauma (ears, sinuses), oxygen toxicity (CNS or pulmonary), fire hazard, and claustrophobia. Contraindications include untreated pneumothorax, certain chemotherapy drugs, and uncontrolled seizures. Topical oxygen risks are minimal but include skin irritation. Our comprehensive screening and monitoring minimize these risks to <1% significant complications.
Topical oxygen delivers oxygen directly to wound surface without systemic effects, suitable for outpatient/home use with fewer contraindications. HBOT achieves higher tissue oxygen levels throughout body, more effective for deeper infections and systemic conditions. They are often complementary: HBOT for acute/severe cases, topical for maintenance or mild-moderate cases. We select based on specific clinical scenarios.
